Health History Form

Please complete the form below and bring to your consultation.

Consultation

Your first visit to our office allows us to get acquainted with you and for you to learn more about us, so we dedicate as much time as is necessary to discuss your questions and concerns and also to diagnose your orthodontic problem.

When you arrive, we will welcome you to our office and assist in taking your medical and dental history. We will then take photographs of your face and teeth. The doctor will then examine your teeth, oral soft tissues, jaw joint, and profile, as well as study how you breathe, swallow, and bite.

After the evaluation is complete, we are always excited and delighted to discuss how your specific orthodontic wishes and goals can be incorporated into treatment options designed to achieve the ideal treatment outcome.  The details of the treatment, such as length of treatment, fees, and payment options, are then explained so you can make a suitable choice and also understand how we deliver quality care that results in a happy, healthy smile!

Records

Complete diagnostic records are made consisting of plaster study models of the teeth and X-rays. These records are also helpful in tracking the progress of treatment as the teeth move under orthodontic care.

Regular Appointments

The appointment when you get your appliances (braces, expanders, etc.) usually takes from one to two hours. You’ll then see us at regular intervals 6 to 10 weeks apart for adjustment, which takes 20 to 40 minutes. We work flexible office hours from Monday to Friday. Patients are seen by appointment only. We make every effort to be on time for our patients, and we ask that you extend the same courtesy to us. If you cannot keep an appointment, please notify us immediately. Cancel only if it is an emergency. It is difficult  to reschedule you since most appointments are made 6 to 10 weeks in advance.

Payment & Office Policies

Cost

Financial considerations should not be an obstacle to a beautiful smile. Orthodontic treatment is an excellent investment in the overall dental, medical and psychological well being of children, adolescents and adults. We understand that families and individuals have different budgets and needs in fulfilling their financial obligations; therefore, we offer several payment options to fit these needs and will be happy to discuss these payment options with you. The cost of treatment will vary, depending on the severity of the problem and so our treatment coordinator will work out a plan suited to your individual needs before treatment begins.

Fees

In an effort to keep orthodontic fees down while maintaining the highest level of professional care, we have established this financial policy: If full payment is made at the onset of treatment, we will offer a fee discount. To fit your individual needs, financial arrangements can be made to extend your payments over a period of months. For your convenience, we accept payments by VISA, MASTERCARD, DEBIT CARD, and CHEQUE.
